We are looking for a part time Care Co-Ordinator to join our growing and friendly team!
As a Care Co-Ordinator, you will play an integral part in managing the daily communications of our site. You must be personable and discreet as you will be liaising with the entire team to ensure all our valued user members care needs are met. This is a fast-paced role and requires someone who is dedicated to providing a high-quality level of service to the local community and is able to remain positive and professional when working under pressure.
The role details:
Location: Colne Valley, Huddersfield
Salary: £22,000 to £24,000 pro rata – plus one call
Start Date: Immediate after notice period
Hours: 16 hours per week – preferably worked over 4 days
On Call: Weekend on call, 1 in 3, some weekday and to provide holiday cover as and when required
Duties will include:
· Working effectively alongside colleagues within the co-op to ensure safe, effective and efficient delivery of a flexible, high quality service
· Ensuring continuity of delivery of high quality person centred care, including rostering
· Being accountable to the Registered Manager for the smooth running of day to day user member communication
· Assisting the Registered Manager in all aspects of managing the service provision
· Ensuring that safeguarding incidents are reported in accordance with policies and procedures
· Identifying training and development needs of staff members
· Planning ahead to cover holidays, training and planned absence
· Following policy and procedures in respect sickness and leave
· Carrying out spot checks, supervisions and quality reviews
· Ensuring support and care plans are reviewed and updated
· Conducting risk assessments
· Attend, leading and contributing to staff and Board meetings as required
· Delivering care when required
· Establishing community links for user members
